Annual ‘REACH for all’ Conference Focuses on Supporting Self-Management in Chronic Disease

(MONCTON) June 20, 2014 - Horizon Health Network will host the 'REACH for all' conference on June 24 at the Delta Beauséjour. The conference is an annual event for health-care professionals who are passionate about promoting good health and supporting those living with chronic conditions. 

More than 200 participants from across New Brunswick and the Atlantic Provinces will meet and exchange information around this year's theme of 'Supporting Self-Management in Chronic Disease.'                                

"Chronic health conditions such as heart disease, emphysema, diabetes, arthritis, and depression are on the rise. These conditions require ongoing care for months and years as they often cannot be cured," says Denise Connors, Manager, Chronic Disease Prevention and Management with Horizon Health Network and conference co-chair. 

The conference is designed to provide participants with knowledge and skills for use in day-to-day practice. "People with chronic health conditions may only spend a few hours a year with health-care professionals; the rest of the time they care for themselves. Physicians and other professionals have a key role to play in supporting patients and their families in learning what they need to know about their conditions. Just as important, is gaining confidence and sustaining motivation to set and meet goals that are important to them." says Ms. Connors. 

The keynote address will be delivered by Dr. Michael Vallis, Registered Psychologist with the Behaviour Change Institute, Capital Health, Nova Scotia. According to Dr. Vallis, "Self-management support is based on developing a trusting, collaborative relationship between the health-care provider and patient. A strong partnership is essential to supporting people to find the motivation and strategies to change and maintain health-related behaviours like quitting smoking or getting more active."

The REACH for all conference respects Horizon Health Network's Healthy Meetings and Events Policy. Opportunities for physical activity, and healthy meals and snacks will be provided. Thanks to the unique partnership between Horizon and the Delta, the menu will focus on supporting local food producers.
