What is Palliative Care?
Horizon's Palliative Care teams help to prevent and relieve suffering and support the provision of the best possible quality of life for our patients and their families. This includes providing physical, social, emotional, practical and spiritual support for the person living with an advanced illness.
Palliative care differs from hospice care in that its aim is to alleviate symptoms of serious illness regardless of prognosis and with or without ongoing health-giving treatment.
Horizon Health Network and its team of health professionals offers Palliative Care through a team of health-care professionals that includes: nurses, physicians, pharmacists, social workers, psychologists, spiritual counselors/chaplain, volunteers and other professionals as needed, such as therapists.
