The Provincial Antenatal Record is a tool to facilitate the assessment of mothers and babies and documentation of relevant information pertaining to women's health, pregnancy care and perinatal care in a standardized matter. It is a tool to aid in the communication between health care providers and provides a guide to evidence-based components of prenatal and maternal care. This form is to be used by all physicians and health care providers delivering perinatal care in the province.
Specific fields in the Antenatal Record will be coded and collected as part of the perinatal data chapter. The collection of data will help identify and develop standards, best practices, health care initiatives and evidence-informed policies.
