Working Better Together: The Prevention & Management of Chronic Obstructive Pulmonary Disease (COPD)

To deal with modern day health challenges, health-care professionals are always reaching further to equip themselves with the right knowledge, skills and tools to provide the best possible services to the patients and clients they serve.  The annual REACH conference is designed to support this effort by providing an affordable professional development opportunity that gives participants a strong knowledge base on relevant topics and arms them with skills and tools applicable to their day-to-day practice.

This year, REACH will be offering a program designed to highlight evidence supporting an integrated approach to chronic disease management and the benefits of integrated care in COPD prevention and management.  Topics covered will include an overview of integration concepts, the prevalence and cost of COPD in our system, COPD outreach for patients and families, smoking cessation for vulnerable populations and interdisciplinary team approaches to self-management and transitions.

Who Should Attend?

Health-care providers and social services professionals invloved in the prevention and management of chronic disease through clinical and support services (e.g. hospitals, extra-mural, community health, public health, primary care, ambulatory care, mental health and addictions and non-government agencies).
