Horizon receives a score of 97% from Accreditation Canada

Dear Staff, Physicians and Volunteers,
Thank you for participating in and supporting various Horizon Health Network teams who recently welcomed sixteen surveyors from Accreditation Canada this past September.
As part of the accreditation program, the organization underwent a rigorous evaluation process. Following a comprehensive self-assessment, 16 surveyors from across Canada conducted 155 survey visits at 30 facility sites. During these visits, the accreditors assessed leadership, governance, clinical programs and services, comparing them against Accreditation Canada requirements for quality and safety.
We are pleased to report that all the hard work and preparation by staff, physicians and volunteers has resulted in Accreditation Canada awarding Horizon Health Network with an accredited four-year status.
Over the course of a week, through observations, discussion and documentation, the accreditation team evaluated 3,269 separate criteria. Horizon successfully met 3,157 of those criteria, or 97 per cent.
Accreditation Canada found our overall strengths to be:
- Our People - board, leadership, staff, physicians and volunteers
- Patient Focused Care - sensitive to the populations served
- Great team work with commitment to collaboration
- Strong community partnerships
- Access to primary and community care
They also pointed out some overall areas for improvement, which were:
- Continue to build the foundation to ensure ongoing patient-centered care
- Integrate Information Technology
- Continue to standardize care and care processes
Accreditation Canada is an independent, not-for-profit organization that sets standards for quality and safety in health care, and accredits health organizations in Canada and around the world. This evaluation assists health care organizations to recognize their strengths, identify areas for improvement and to create a plan of action to improve the needs of patients, families, and communities.
We certainly believe this milestone is worth celebrating. On behalf of the Board and the Executive Leadership Team, we congratulate every employee, physician, volunteer, and community partner for their continued commitment to providing safe, quality health care to our patients.
